Gathering or remembering your entire health history can be an overwhelming task. Thankfully, several resources are available to help you keep track of everything from vaccines and diagnoses to prescribed medications and lab test results.
How to Keep Track of Vaccinations
Because vaccination may be required for certain jobs, travel abroad, or school registration, it's important to keep a record of the vaccines you get. Many healthcare providers and clinics participate in an immunization registry. Ask your doctor, pharmacist, or vaccine provider for an immunization record form and request that your vaccines be well documented.
Recent graduates can check with your high school or college health services for dates of any immunizations. Generally, records are kept only for 1-2 years after a student leaves the system.

Mobile Apps for Personal Medical History
Imagine how great it would be to have a clear picture of your entire health history at the touch of a button. With the help of technology, you can! Here are a few mobile apps that provide secure and convenient access to your medical records:
- Picnic Health puts patients in control of their health by collecting and digitizing their medical records into a PicnicHealth timeline.
- Hixny, an online health care portal for patients, brings together all of your health information in one place so you can see what your providers see—as long as you’ve given them consent to share your information.
- Apple Health App for iOS gathers health data from your iPhone, Apple Watch, and apps that you already use, so you can view all your progress in one convenient place. Additional add-ons and integrations allow for secure storage of personal health information.
Please note: there is no standard HIPAA 'certification' for mobile apps. However, the services you use should be able to show proof of third-party audits that were completed to assess the protections in place. Do your research and carefully read all terms associated with the app and device that is accessing your personal information.
Today, it's not uncommon for physicians, clinics, and pharmacies to encourage patients to request appointments, access records, or even pay their bills online. When available, taking advantage of these types of services is a great way to keep track of your medical history. Equally important, it improves communication and can significantly reduce the risk of error in sharing important information about your medical care.
